Vaginismus Treatment in Pittsburgh
Vaginismus is the involuntary tightening of the pelvic floor muscles when something tries to enter the vagina, whether that is a partner during intercourse, a tampon, a menstrual cup, or a speculum at your OB appointment. The muscles essentially clamp shut without you telling them to, and the result can range from intense burning or stinging to a feeling that penetration is simply impossible. It is more common than most women realize, and it is highly treatable with the right care.
At Form and Function Pelvic Health in Pittsburgh, we work with moms and younger women who have been struggling with vaginismus for months or even years, often without anyone explaining what is going on or offering real solutions. You are not broken, you are not alone, and there is a clear path forward.
What causes vaginismus?
Vaginismus can show up for a lot of reasons, and often it is a combination of factors rather than one single cause. Some of the most common contributors we see in our Pittsburgh practice include:
- Pelvic floor muscle tension that has built up over time, often without you realizing it
- A history of painful intercourse, pelvic exams, or tampon use that taught your muscles to brace
- Hormonal changes during postpartum, breastfeeding, or perimenopause that thin vaginal tissues and reduce lubrication
- Scarring from perineal tearing, an episiotomy, or a C-section
- Anxiety, fear, or stress related to intimacy, sometimes tied to past experiences
- Other pelvic conditions like endometriosis, interstitial cystitis, or vulvodynia
Vaginismus is a physical response, not something you are doing on purpose, and it does not mean anything is wrong with you. Your nervous system and pelvic floor have learned a protective pattern, and pelvic floor therapy helps your body unlearn it.
Common Symptoms of Vaginismus
Every woman experiences vaginismus a little differently, but these are the symptoms we hear about most often:
- Burning, stinging, or tearing sensation with attempted penetration
- Feeling like there is a wall or a block at the vaginal opening
- Inability to insert a tampon, menstrual cup, or finger
- Pain or impossibility with speculum exams at the gynecologist
- Pain that makes intercourse difficult or completely off the table
- Anxiety or dread leading up to intimacy
- Pelvic floor muscles that feel tight, tender, or sore
If any of this sounds familiar, you are not alone, and you do not have to keep pushing through or avoiding it.
How Pelvic Floor Therapy Treats Vaginismus
At Form and Function Pelvic Health, treatment is always built around what your body actually needs. Care for vaginismus typically includes:
- Pelvic floor muscle neuro re-education to slowly teach your muscles to tolerate touch and penetration again. This often includes graded use of a dilator or pelvic wand at a pace that feels safe for you.
- Manual therapy, including trigger point release of the internal pelvic floor muscles where tension and tenderness live, plus fascia release of the abdomen, back, hips, and external pelvic area.
- Relaxation and breathing training to help your pelvic floor and surrounding muscles learn how to let go, since the pelvic floor cannot release if the rest of your body is bracing.
- Behavioral and lifestyle guidance, including recommendations for better lubricants and vaginal moisturizers, retraining the thoughts and habits leading up to intimacy, and tools like the Ohnut to reduce depth of penetration as your body builds tolerance.
- A tailored home exercise program to target your specific areas of tightness, weakness, or coordination so progress keeps moving between sessions.
Vaginismus responds really well to consistent, skilled care, and most of our Pittsburgh clients are surprised by how much progress they make once their pelvic floor finally gets the right kind of attention.
